Applying new National Identity Card for the first time

The applicant should be a Sri Lankan citizen.

National Identity Card Application Form should be obtained from the Divisional Secretariat or Grama Niladari.

Every citizen who completed the age of 15 years should apply for a National Identity card.

If the applicant has completed the age of 16 years, the applicant has to pay Rs. 250.00 of fine to the government as a late charge.

Should attach the original birth certificate certified by Additional District Registrar and a photocopy.

Should attach 'photograph reference' taken from the Department for Registration of Persons approved studio.

Should paste paid receipt of Rs.100.00 to the Grama Niladari on the 5th page of the application.

Should attach envelope with Rs.110.00 stamps without writing name and the address.

The completed application should be handed over to the Grama Niladari

Should attach the original birth certificate issued by the country of birth or the consular birth certificate issued by the Sri Lankan Embassy of the country of birth.

National Identity Card Application Form should be obtained from Divisional Secretariat or Grama Niladari.

If dual citizenship is more than 6 months, the applicant has to pay Rs. 250.00 of fine to the government.
